Research background
Amylin is a peptide hormone co-secreted with insulin by pancreatic beta cells. Cagrilintide is a 37-amino-acid analogue with a disulfide bridge and a twenty-carbon fatty acid chain, a modification that favours albumin binding and lengthens its half-life. It acts as a non-selective agonist of amylin and calcitonin receptors.
It is one of the most extensively trialled molecules in its group: a phase 1b trial with semaglutide (Enebo et al., Lancet 2021), a phase 2 dose-finding trial (Lau et al., Lancet 2021) and a later programme of randomised trials of the combination known as CagriSema. In the laboratory it serves as a reference for studying signalling in the calcitonin receptor family.
Reconstitution and handling
- Work on a clean surface and disinfect the vial stopper with an alcohol wipe before piercing it.
- Add the bacteriostatic water slowly, letting it run down the inner wall of the vial rather than landing on the powder.
- Do not shake. Swirl the vial gently until the lyophilized powder has fully dissolved: vigorous agitation degrades the peptide.
- The solution should come out clear. If it turns cloudy or shows suspended particles, do not use it.
Storage and stability
- Before reconstitution: refrigerated between 2 and 8 °C, protected from light and moisture.
- After reconstitution: in the fridge and away from light. In solution the peptide lasts far less than as a powder, so it is best used soon.
- Avoid freeze-thaw cycles.
